Booking an appointment for your COVID-19 test

Please note:

·       By booking your appointment online, you acknowledge and agree to receive your appointment information by email.

·       Your personal information is being collected through this site by the COVID-19 Assessment Centre you select and will be used only to register your appointment and to send your appointment confirmation by email.

·       You also continue to have the option to call us at 905-378-4647, ext. 42819 (4-CV19). Staff at the Assessment Centre will return calls as soon as possible to schedule a date and time for testing.

·       If this is a medical emergency, call 911 or visit the nearest Emergency Department. Advise them of your symptoms and if you have recently travelled.

Important reminders for those coming to our Assessment Centres for testing:

  • If you are an Ontario resident, you will need to bring your Ontario Health Card to the Assessment Centre with you when you arrive for testing. If you do not have an Ontario Health Card, please bring another piece of identification.
  • Please wear a mask.
  • For safety reasons, Assessment Centres have separate entrances from the hospital. Those being tested for COVID-19 are asked not to arrive at the main entrance or through the Emergency Department.
